VelocityCare Referral Coordinator goes the extra mile

Submitted by jbgreer on

Latavia Craghead well deserves a nod of recognition. Latavia works as VelocityCare's referral coordinator. When setting up a referral for a patient in need of services today, she came to a bit of a roadblock, but she didn't let that stop her commitment to ensuring that the patient received the care she needed. She had to think outside the box and look for other options to meet the patient's needs. I am grateful that she took the time and effort to care for this patient without letting roadblocks prevent the patient from receiving the follow-up care needed.

Back in the Spring of 2022, Community Health and Outreach was providing Sun Safety education at weekly Walk and Talks throughout our service area. During one of these walks a flex staff member, Chris Maxwell, noted an oddly shaped mole on a participant’s leg. At the end of the walk Chris approached the participant and highly encouraged them to visit their physician or dermatologist as soon as possible to get the area checked. The participant acknowledged Chris’s concern and accepted her advice.

Amazing Patient Care!

Timothy Magee went above and beyond with a patient we both had on 8S who required some extra assistance. He responded to every call bell and phone call (and there were MANY) in a timely manner and helped to calm the patient when they were feeling anxious and upset. From the time we got here he was ready on his feet and was both reassuring to the patient and to me when things were feeling difficult. His patient first attitude shines and I feel fortunate to have had the ability to work alongside him for the good of our patient!

Great Teamwork!

Virtualist Program is a Success

Dr. Dunn, the remote provider who began seeing patients in October has seen nothing but success with this program.  He works 4 days a week and is at 100% capacity most of those days.  He is currently seeing primarily same/next day acute visits and limited hospital follow ups.  Dr. Thompson, another remote provider, fills in when needed and is also seeing great success.
