Shine Nomination Deadline Extended to June 13

We know you are busy, so we are giving you an extra week to apply for this year's Quality Awards or nominate someone for the Values Awards, Provider Excellence Awards or President's Award. 

How To Apply

• Quality Awards: Visit the Quality Awards page to download the application template. Complete the application and submit it through this Microsoft form.
• Values Awards: Visit the Values Awards page to download the application template. Complete the application and then submit it through this Microsoft Form.
• Provider Excellence Awards: Visit the Provider Excellence Awards page to download the nomination form. Complete the form and then submit it through this Microsoft form.
• President's Award: Complete the President's Award webform.

Award winners and finalists will be announced at the Shine Awards ceremony on Monday, Nov. 7. We are planning an in-person event this year!
