Tobacco-Free Incentive

We offer a tobacco-free incentive that could save you money on your 2021 medical benefits premiums. The incentive is $50 per pay period. 

To receive the incentive:

• If you and everyone you cover on your medical plan are tobacco-free, confirm your tobacco-free status when you complete your benefits enrollment.
• If someone on your medical plan uses tobacco, you may still be eligible for the incentive if the tobacco user takes one of these steps:
  • Enrolls in the Quit for Life program, which is a tobacco-cessation program that is offered at no cost to Carilion employees and their family members.
  • Enrolls in another type of tobacco-cessation program that you pay for.
  • Provides a doctor's note stating that it is medically inadvisable or unreasonably difficult to be tobacco-free.
• If you choose an alternative method to receive the tobacco-free incentive, you must submit two forms to Human Resources:
  • The Tobacco Use Certification and
  • The Qualifying Alternative for the Tobacco-Free Incentive Form
  • Both forms and doctor's note, if applicable, need to be submitted to Human Resources by email at HRServiceCenter@carilionclinic.org or fax at 540-857-5209.

During the year, fill out and return both the Tobacco Use Certification and the Qualifying Alternative for the Tobacco-Free Incentive Form to Human Resources by the 15th of the month to begin receiving the incentive the following month.
