Many thanks to Josh Sparks for going out of his way to physically show me where I needed to go for a meeting at the Rehab building!

Doing this made Josh late to his own meeting at RMH.  This is only one of many examples of the selflessness that Josh exhibits and I hope he knows it doesn't go without notice!

Great Catch by Brook Reeves RN at CFMH ED

Brook Reeves, RN with CFMH's ED noticed a supply discrepancy during code cart check off.  Brook addressed this issue and had the courage to speak up and take action.  Brook's attention to detail and work to get the issue resolved keeps patients safe at CFMH.

Great Catch!!! Elise Taylor’s awareness prevents harm for patients.

Elise Taylor RN, 8MICU, great awareness leads to a Great Catch!  Elise noticed that an incorrect fluid was set to go in the CRRT machine.   The incorrect fluid could have easily been overlooked and potentially damaged the CRRT machine.  If Elise did not notice this, a patient may have been harmed while using the CRRT machine. 

Pharmacist's Unsafe Device SafeWatch Report Leads to a Class I FDA Recall

Jessica Schad, a Pharmacist at Carilion Roanoke Memorial, reported an unsafe device concern in SafeWatch.  In addition to Jessica's report, two additional reports were received leading to a voluntary report to the FDA by Carilion's Patient Safety Team. The FDA recently announced that Jessica's report along with other similar reports lead to a Class I Recall, the highest and most urgent recall possible.    

CARES Presurgical Testing RN Reports a Great Catch!

Dawn Ketterer RN with CARES Presurgical Testing recently reported a Great Catch in SafeWatch.  Dawn recognized that the wrong laterality had been posted on the Master Daily Schedule for the patent's surgery and corrected the error.  This is an example of the layers of safety checks prior to surgery working to keep our patients safe!  Thank you Dawn for the COURAGE to speak up and report this Great Catch.  This report will be in the running for the February's Great Catch Award!  

CRMH VICU RN Reports a Great Catch

Ivy Pendleton RN with CRMH VICU recently reported a Great Catch in SafeWatch.  Ivy noticed an unsafe medication stocking issue that could have potentially led to a patient event.  Thank you Ivy for the COURAGE to speak up and report this Great Catch.  This report will be in the running for the February's Great Catch Award!
