Help Improve Patient Questionnaires in MyChart

When it comes to our patients’ health care needs, those who work one-on-one with our patients know best. That's why the MyChart steering committee is asking for input from our clinical departments to help update and create new patient questionnaires in MyChart. 

We use questionnaires to assess patients prior to visits, giving our care teams a better look at patient health before appointments. The questions that each patient receives vary based on the type of visit. 

Until recently, we collected information about patient medical history and severity of symptoms verbally, then submitted it in different formats for the MyChart steering committee to organize and review. Implementing a new process to have questionnaire responses collected in MyChart will streamline things and enable staff to focus more on care rather than on paperwork and administering questionnaires. 

“Employee input is especially important to help refine our questionnaires to improve efficiency and overall patient care,” said Tom Miller, senior director of Ambulatory Services. “Capturing this data helps us understand patient needs and improve the quality of the care we provide.”

MyChart makes questionnaires available to patients up to seven days prior to an appointment. To review existing questionnaires within Epic:  

  • Select Rooming.
  • Navigate to the Screenings activity. 
  • Scroll down to MMRC (Modified Medical Research Council). 
  • Click New Reading
