CGCH Care Transitions Team Takes Patient Caring to the Next Level !

Amanda Terry, Pharm.D,  Marianna Gomez De La Espriella, M.D,  and Drema Gautier, Social Worker know the struggle that our patients have to afford expensive, life saving medication at discharge!

Together they worked as a team to ensure that a patient would be able to obtain a critical and extremely expensive medication from the retail pharmacy before leaving our facility.  This was accomplished by ensuring that the prior authorization was completed before discharge.  It wasn't a simple task.  It took time.  And, the team prevailed for the patient.  Thank you!
