Community Grants Support a Healthier Southwest Virginia

We have awarded 19 community grants to nonprofit organizations across our region as part of the 2026 Community Grant Cycle. These partnerships support programs focused on key health priorities identified through our Community Health Assessments. Our 2026 grants center on organizations addressing 3 priority areas:

1. Access to health care and navigation
2. Mental health and substance use
3. Food insecurity and nutrition 

These investments help extend our mission of improving the health and well-being of the communities we serve. We are proud to support the efforts of the following organizations working to connect people to care and resources, improve access to nutritious food, and support individuals experiencing health-related social needs. 

Access to Health Care and Navigation

• Blue Ridge Literacy (Roanoke): Improving health literacy and helping individuals navigate health and social service systems.
• Bradley Free Clinic (Roanoke): Expanding access to primary care and chronic disease management. 
• Commonwealth Catholic Charities (Roanoke): Providing health care navigation for vulnerable populations, including refugee and immigrant communities.
• Free Clinic of Franklin County (Franklin): Increasing access to care, medications, and chronic disease services for uninsured adults. 
• Rockbridge Area Health Center (Rockbridge): Supporting primary care, behavioral health, and insurance enrollment services. 
• Rx Partnership (Regional): Connecting patients with free and low-cost medications. 
• United Way of Virginia’s Blue Ridge (Franklin): Supporting community health worker services focused on care connection and navigation. 
• Virginia Dental Association Foundation (NRV): Expanding access to dental services. 

Mental Health and Substance Use

• Family Service of Roanoke Valley (Roanoke): Expanding counseling and behavioral health services for uninsured and underinsured individuals. 
• Virginia Harm Reduction Coalition (Roanoke): Supporting harm reduction services and linkage to care. 

Food Insecurity and Nutrition

• Agape Center NRV (NRV): Supporting food distribution for rural communities. 
• Appalachian Sustainable Development (Tazewell): Advancing food access efforts that align with food-as-medicine strategies. 
• Blacksburg Interfaith Food Pantry (NRV): Increasing access to fresh produce and nutritious foods. 
• Feeding Southwest Virginia (Roanoke): Connecting food access with chronic disease management programs.
• Giles County Christian Service Mission (Giles): Connecting food assistance with chronic disease education and monitoring. 
• LEAP (Roanoke): Expanding access to fresh food through food-as-medicine initiatives. 
• Plenty (NRV): Improving access to food and nutrition resources in rural areas. 
• Rockbridge Area Relief Association (Rockbridge): Providing food assistance to households experiencing food insecurity. 
• Stepping Stone Mission (Franklin): Increasing food and nutrition access.
