Susie Bell, HAA at CFMH was instrumental in assisting case management with discharging a patient home. The patient reported to the case manager late afternoon that she had no power at home. The MD had placed a discharge order for the patient to go home; however, the patient wears oxygen. The case manager contacted the electric company to obtain a medical certification form. The electric company was to send the form but had a deadline for reconnecting service, so if the form was not returned by the deadline, the patient couldn't be discharged home. After waiting several hours and pacing back and forth upstairs to the fax machine, the case manager discussed with the patient that if the form was not received by the deadline, expect a discharge for the following day. Knowing the situation, Susie volunteered to assist. She contacted the electric company, obtained the form, refaxed it, and then contacted the patient's neighbor to ensure power had been restored. Susie was able to help get this patient discharged home promptly after her efforts - hats off to Susie Bell!!
