Collaboration in General Pediatrics- Tanglewood

Phone RN, Holly Peery, received a call about a patient needing to follow-up with their PCP, Dr. Henchel, after an MVC that significantly impacted the patient's health and normal life. Holly made the appointment and notified the PCP, notified the CTL that the patient would be arriving by ambulance in order to plan accordingly, and provided rehab information to a Gen Peds HAA in order to request records. Because Dr. Henchel will be leaving Carilion in just a few short months, she looked ahead to Dr. Drake's schedule and found an appropriate appointment time that she could see the patient with Dr. Drake in order to make an easier transition of care. This collaboration and teamwork will positively influence this patient's adjustment to a new provider, as well as their new "normal" day to day life.
