Mobile Unit Launch = Real Impact!
On December 17, our Infectious Diseases (ID) team launched a new hepatitis C mobile unit and it’s already making a big impact. In the first few outings, the mobile unit helped 10 patients connect with an ID provider and supported medication delivery and teaching at home.
By bringing care directly to patients, this effort reduces barriers like transportation, time, and stigma. Huge shoutout to Jillian Smith, LPN, for leading this work and building trust with patients every step of the way.
This initiative is part of the ReLink grant which is funded by the Center for Disease Analysis Foundation. It falls under Carilion’s Hepatitis C Care Program, which is led by Dr. Mariana Gomez, managed by Mandy Swann, MPH, and implemented by Crystal Chitwood, LPN and Jillian Smith, LPN with support from the community health workforce.
Together, this team is expanding access, strengthening engagement, and delivering care with compassion—one home visit at a time.
