A huge shoutout to our Population Health and FCM teams for their partnership and dedication to improving blood pressure control across our clinics!

Special recognition to Isaac Mallenbaum for his outstanding leadership in sharing monthly performance data with clinic leaders and partnering with teams to develop impactful PDSAs that drive improvement.

We also want to recognize DeeDee Davis, CTL at IM Hardy, for her creativity and commitment to quality care through implementation of a new creative workflow, updating staff education, and monitoring of second blood pressure readings to ensure patients receive the most accurate assessment possible.
