Case management makes dreams come true!

Kendra Case, case manager for 10 south palliative unit, collaborated with the Carilion transport team to honor a patient's last wish. The patient wanted to see the store that he had owned and loved, and Kendra discussed this wish with the transport team. The transport team agreed to take him by his store on the way to his home, where he will then be enrolled with Carilion Hospice. The gratitude and appreciation from the patient and his loving family brought smiles to everyone's day.
