Carilion Mental Health Makes Stellar Improvements in Quality
Carilion Mental Health had a plan to bolster their Quality program, and a review of their required reporting was at the top of the list. They focused on strengthening their Transition Record (TR-1) Core Measure – a process for documenting patient discharge information in the After Visit Summary (AVS).
They discovered the AVS has two frequent incomplete measures: "Reason for IP Psychiatric Admission” and “Advance Directives.”
After much discussion and collaboration with Chart Abstraction Partners and the System Quality Team, Carilion Mental Health quickly experienced these remarkable results:
- May 508% compliance increase compared to April
- June 15% compliance increase compared to May
- August 91.6% compliance increase compared to July
These are outstanding results, especially when the bulk of the work occurred between late April – June!
