We're Updating the Code Status Language in Epic

We're making updates to the code status language in Epic to reduce confusion and make it clear when the patient's advance care planning wishes need to be documented.

In Epic, code status refers to a patient's wishes regarding resuscitation during a medical emergency, particularly cardiac or respiratory arrest. It's a set of instructions for the medical team indicating what measures to take, or not take, to try and revive a patient.

What is Changing?

Beginning Saturday, June 28: "Assume CPR"—the current placeholder for when a patient’s code status is not active in Epic—will be replaced with "Not on File."

"Assume CPR" previously appeared in the storyboard, status boards and tech work lists. The updated language—"Not on File"—will appear anywhere that the code status is displayed in Epic.

Why the Update?

The current placeholder—"Assume CPR"—has caused confusion across inpatient and outpatient settings and contributed to patient safety events. Properly documenting a patient’s code status is essential to ensuring that their care preferences are understood and respected.

This new language clearly indicates that the patient’s code status needs to be clarified and documented properly. The "Not on File" code status should prompt a conversation with the patient about their code status.

Important Reminder

In the event of a cardiopulmonary arrest, if the code status is listed as "Not on File" and no other information is available, the care team should proceed with CPR until clarification is obtained.
