VIDEO: Reporting Suspected Abuse or Neglect

All Carilion employees (not just those who are designated as mandatory reporters) are responsible for reporting suspected patient abuse or neglect.

If you suspect a patient is suffering from abuse or neglect:

  • Staff: Report it to your manager and notify either the local social services agency or the state abuse/neglect hotline. The hotline for child abuse/neglect is 800-552-7096 and the hotline for adult abuse/neglect is 888-832-3858.
  • Managers: Make sure the patient is safe and submit a SafeWatch report if the suspected abuse or neglect happened at a Carilion facility.
  • Document these conversations in the patient’s medical record.
