Above and Beyond for Patient Safety

I work on the admission discharge team and while doing an admission and going over pre admit medications the patient stated to me that this was his dad's chart and not his I was reviewing. He was a Jr. and this seemed perfectly reasonable to me since he claimed out of several meds he only took one. I had no idea what to do and I was very concerned that his name was on the wrong chart. My back up that day was Cindy Blackburn, when I related what was going on she came almost immediately and started digging through the chart and calling to get the information fixed and clarified. this was a timely process that required a lot of patience and perseverance. In the end, Cindy clarified that this was indeed this patient's chart and we got the meds that he was not taking deleted. She would not give up until she was assured that the information in this chart was correct. This is COMMITMENT at its best.!!
