Help Shape the Future of Our Communities’ Health
Your voice can help shape how we understand and respond to health needs across our region. By completing the Community Health and Well-Being Survey, you can share what matters most to you and help inform efforts to improve the health and well-being of our communities.
What is a Community Health and Well-Being Survey?
The health concerns of the communities we serve can change over time. That's why we conduct Community Health Assessments every 3 years. They help us uncover issues, show us where changes are needed, and track and promote the progress we make in key areas.
The Community Health and Well-Being Survey is conducted as part of our Community Health Assessments to better understand the strengths, needs, and barriers that affect health across our region. Our Community Health and Development team works alongside local organizations and community leaders to gather and analyze information from the communities we serve.
What we learn through this process helps Carilion and our community partners identify health priorities and inform future programs, partnerships and investments.
Why are Community Health and Well-Being Surveys Important?
Understanding the needs and resources within our communities helps Carilion and other health-focused organizations plan programs and services that can make a meaningful impact. Findings from previous surveys have helped shape a variety of community initiatives. A few examples since the 2024 assessment:
- The Tazewell Health Collaborative, which strengthens collaboration among community partners and coordinate efforts to address the health priorities identified for Tazewell County. The collaborative organizes its work through groups focused on chronic disease, mental health and substance use, and injury and violence prevention.
- Melrose Plaza, a community hub in Northwest Roanoke that brings together health and wellness resources, education, food access, financial education, arts and other community services. Carilion is a key partner in Wellness on Melrose focused on increasing access to health resources, empowering healthy behaviors, and supporting healing.
- Southwest Produce Prescription (SWPRx), a six-month program developed with community and healthcare partners to connect eligible Medicaid patients with healthy food and nutrition support. Early results show improvements in food access, healthy eating and blood sugar levels.
