What You Need to Know About Conflict of Interest Questionnaires and Education

Our Conflict of Interest (COI) Questionnaires are one way we uphold our organization's Code of Excellence and avoid situations that could harm our reputation, finances, or tax-exempt status.  

We send the COI Questionnaire and related education each year to employees who may influence operational decisions, and to others with positions where additional review may be appropriate based on potential risk considerations and trends. 

Who Will Receive the Questionnaire and Education Assignment?

In general, you will receive a COI Questionnaire and an education assignment if you work in one of these roles:

  • All leadership roles 

  • Physicians and advanced practice providers 

  • Case managers and social workers 

  • Physical, occupational and speech therapists 

  • Licensed counselors  

  • Internal auditors, Compliance, and Privacy

  • Key personnel for research projects 

  • TSG and Health Analytics

  • Employees who make decisions involving material purchases of goods or services for Carilion

  • Individuals with certain specialized knowledge/skills who manage proprietary information

Some employees in other roles may receive the questionnaire and education assignment as well. Employees who already completed the 2026 questionnaire this year will not receive it again—this includes board members, some leaders, the Pharmacy and Therapeutics committee and subcomittees, and the Value Analysis committee. 


COI Questionnaire Key Things To Know

New for 2026: Previously, your responses in the questionnaire were pre-populated from the prior year for your review. However, due to extensive updates aimed at improving clarity and streamlining many questions, all responses must be completed in full this year. Thank you for your understanding and cooperation. 

If you are required to complete the questionnaire, it will be sent to your Carilion email address from Carilion Clinic Compliance <noreply@coi-smart.com>. 

  • The subject line will be Conflicts of Interest Questionnaire. 

  • If you do not see it in your email, check your Other email box. 

  • Click on the link within the email and log in using your Carilion username and password. 

  • The deadline to complete the questionnaire is Wednesday, May 20.

  • If you do not receive a COI Questionnaire and work in one of the roles listed above, contact the Compliance team at compliance@carilionclinic.org.


COI Education Key Things To Know

If you receive the questionnaire, you will also be assigned education in Workday. This is a separate requirement from the survey. The education is titled Required - 2026 Conflict of Interest (COI). It will be available from your Workday Learning dashboard and should be completed by the deadline listed there.

The 2026 Conflict of Interest (COI) Questionnaire will be sent on Monday, April 20 to employees who are required to complete it. These employees also have a separate, required COI education assignment in Workday.

If you are required to complete the questionnaire, you'll receive a link to it at your Carilion email address on Monday, April 20. The deadline to complete it is Wednesday, May 20.

The COI education is assigned in Workday and will be available from your Workday Learning dashboard. The deadline to complete the education will be listed there.

Employees who already completed the 2026 questionnaire this year will not receive it again.
