Conflict of Interest Questionnaires Coming April 21

In order to operate in a manner consistent with the Carilion Clinic Code of Excellence, protect our non-profit status and meet certain federal regulatory requirements, it is important we avoid conflicts of interest.

To support this effort, each year we send a questionnaire to staff who may influence purchasing, operational decisions at Carilion, or others as deemed appropriate based upon potential risk, to identify potential conflicts of interest.

The questionnaire will be distributed this year on Monday, April 21.

Questionnaires will be sent to:

  • Carilion-employed physicians and APPs
  • Employees in management roles
  • Case managers and social workers
  • Physical, occupational and speech therapists
  • Licensed counselors
  • Internal Audit employees
  • Employees who are considered key personnel for research projects
  • Employees who have responsibility for making decisions involving material purchases of goods or services for Carilion
  • Others as deemed appropriate based upon potential risk

If you are required to complete the questionnaire, you will also receive an education assignment in Cornerstone in April. Here are key things to know:

  • The questionnaire will be sent to your Carilion email address from Carilion Clinic Compliance at noreply@coi-smart.com with the subject line "Conflicts of Interest Questionnaire."
  • To open the questionnaire, click on the link within the email and log on using your Active Directory username and password.
  • Do not click Reply to the message.
  • The deadline to complete the questionnaire is Friday, May 30.
