Great Catch! Keeping Our Patients Safe

Our Great Catch Awards honor employees who detect and report potential errors or process issues before they can impact patients. By identifying these great catches, we enhance our systems and processes across Carilion to keep patients safe. We have presented several Great Catch awards in the past few months:

Kevin Brocker, R.N., CSICU, recognized that pressure bags were loosing air significantly faster than the previous bags that were on back order. Kevin provided education to staff on CSICU. His report in SafeWatch, along with other reports, led to a substitution in product.
Gabrielle Castro, R.N., Megan Miller, R.N., and Melissa Wright, R.N., CRMH PACU, recognized a recent EPIC upgrade caused a change in how often Dilaudid is administered. Their report alerted the Pharmacy where it was quickly corrected.
Nicole Espinosa-Meythaler, R.N.,11PICU, recognized the carb counting chart used on their unit did not include gluten free options (which are considerably higher). Nicole worked to add gluten-free options to their carb counting sheet for accuracy in the future.
Caleb Hamm, R.N., CNRV, prevented a medication error before it reached the patient. The medication order was "Auto verified," but Caleb's attention to detail allowed the pharmacist to give the medication as instructed.
Ashton Richards, R.N., Carilion Pediatrics – Roanoke, recognized an interface issue between EPIC and the Virginia Immunization Information System. The interface was quickly corrected because of Ashton's report.
Jessica Simmons, Radiology Technician, CNRV, discovered a knee x-ray had been ordered on the wrong patient. Jessica's questions about knee pain and a suspected fall led her to realize the error. She contacted the authorizing provider before the error reached the patient.

Our work to improve patient safety never ends! If you identify a potential error or process issue, report it so we can learn from it and prevent any patients from being harmed. If you have a Great Catch to report, there are two ways to report it:

Go online to SafeWatch—access it through the icon on your computer's desktop or through the SafeWatch Reporting hub (formerly called the Event Reporting hub).
Call the event reporting phone number, 540-981-SAFE or 7-SAFE. Calls will be forwarded to the Quality and Patient Safety contact at the facility where the event occurred.
