Home Health OT, Angela Roberts Ensures Patient Safety

A home health patient who was receiving remote patient monitoring (Daily interaction with technology to record BP, heart rate, weights, etc) was alerting in our centralized telemonitoring center a low BP, i.e. 70/40's.  This patient is dealing with many complex illnesses and phone calls placed by the central monitoring nurse in attempts to complete a more thorough virtual assessment were not being answered .  Rhonda Ihlenberg, monitoring nurse, reached out via perfect serve to Angela Roberts, OT who was scheduled to visit.  Angela arrived at the home, assessed the patient and called 911 to have the patient transported to the ED, where he was ultimately admitted to ICU.   While our focus is on preventing unnecessary readmissions, there are times in which patients decompensate and require urgent attention.  I'm thankful that Rhonda had virtual eyes on the patient until Angela could arrive and get him to a safe level of care.  I'm likewise thankful that our care teams act quickly and appropriately to ensure quality and safety for our patients.
