Heather Hall, RN: Home Care Hero!

Heather Hall, RN with CNRV Home Health went the extra mile last week and it truly paid off for one of our patients.  Heather wasn’t able to make contact with a new admission that was assigned to her on that particular day.  She tried to call multiple times.  When she couldn’t get an answer she contacted the discharge planner at a local facility to make sure the patient had been discharged and that she had the right contact information. 

Heather spoke to the patient’s case worker, who said she had spoken to the patient earlier that morning and asked if Heather could “just go by his house to check on him as it was unusual for him not to answer” The caseworker felt like it was a safe location and would be safe for Heather to stop by and see if he would answer the door.  When Heather arrived and knocked on the door, the patient hollered and told her to come in.

Heather found her patient face down in the doorway of the bathroom floor.  He had been there since early that morning, for hours, and wasn’t able to get up. He told Heather that he had "been praying someone would find him and here she was like an Angel."  Heather called 911, stayed with the patient, got him stabilized, reassured him, and kept him calm until the ambulance arrived. The patient was then transported to the hospital.

This is a beautiful example of the heart of our home care staff and the value that Home Health has in serving our community.
