Mackenzie proactively identified an extended‑infusion cefepime order that had been placed ahead of the official rollout and noted that the required concentration was not yet available in the smart pump library. She promptly raised the concern, allowing Pharmacy to confirm that the concentration had been inadvertently omitted during the build. The missing concentration was quickly added, and a mid‑cycle pump update was deployed systemwide.

Thanks to Mackenzie’s vigilance, no patient harm occurred, and the go‑live date for extended and continuous infusion antibiotics was appropriately delayed to ensure safe implementation. Thank you, Mackenzie, for your attention to detail and commitment to patient safety.
