ED Clinical Coordinator is Making Rounds at TCH

Fiorella Chavez-Yost, or Fio as we all call her, is simply put THE BEST!  We started a new initiative with on-site clinical coordinators imbedded in our emergency departments throughout Carilion a little over a year ago.  Fio went onsite in July of 2022 and she is making quite the impact.  Fio has a keen eye for identifying any potential gaps in care.  Fio has made an impact on connecting patients to follow-up care for the TCH ED.  She works tirelessly to connect patients to primary care and when needed, specialty care post ED Discharge.  Fio is flexible and willing to help anywhere she can at TCH and has really become an integral part of the TCH team!
