Over the last 6 months Dan Djuric has pioneered a critical new technology tool for the ambulatory Care Coordination team. This work supports the care of patients served in approximately 50 Family and Community Medicine and Internal Medicine practices across Southwest Virginia. This new technology tool has replaced a manual paper tracking system used to identify patients discharged from Carilion and non-Carilion facilities, who are risk stratified and followed via telephone contact by nurses for thirty days. Because of Dan’s work, hundreds of patients are able to efficiently connect with a member of the Care Coordination team after hospital or other facility based care.

Hospital discharge is a complex transition for most patients, and they often have an incomplete understanding of their medication or treatment regime, making them at risk for readmission. In the development of this new discharge Transition Tracking Tool or “T3”, Dan worked tirelessly to deliver an excellent product. His professional approach was evident as he engaged closely with the end users, designing and redesigning components before and after usability testing and adjusting to evolving workflows. Dan demonstrates a questioning attitude which has made all the difference in this project. He analyzed the T3 data with the managers and recommended adjusting the call frequency, which allowed the staff to reach additional patients. Dan works hard to understand what and how the customer will use the tool to ensure it will meet the need; this dedication has made design and implementation of the T3 data tool a complete success.
