CVI "All-Star" Awards

Lexie Basiliere, RN, EP Lab and Stephanie Trail, RN, EP Lab identified an unexpectedly reduced response to heparin in three separate patients and promptly reported the concern through SafeWatch.

Pharmacy and medication safety leadership were engaged, resulting in:

·          two MedWatch reports were submitted to the FDA

·         the implicated heparin lot was sequestered

·         other facilities were notified

·         the manufacturer’s quality control department was alerted

These SafeWatch reports and the responses to the reports protected patients from a potentially sub-therapeutic lot of heparin.  

We commend Lexie and Stephanie for their vigilance, timely reporting, and using their voice to advocate for patient safety! Thank you for being "All Stars" every day!
