Courage to Speak Up in Palliative Care

One of our nursing assistant's in Palliative Care had the courage to speak up on June 13, 2019 when we received a patient on transfer from another unit at RMH.  As she was starting to chart the vitals in a patient's medical record, she noticed that the name of the patient in EPIC did not correspond to the name of the patient that was transferred into the room.  She stopped what she was doing, having not filed any of her work in EPIC, and asked the nurse to clarify the name of the patient that was physically in the Palliative unit bed.  The nurse spoke the patient's name to the nursing assistant and the nursing assistant informed the nurse that the incorrect patient had been transferred in EPIC.  The nursing assistant deleted the vitals signs that she was charting and the nurse immediately contacted the sending unit to correct the error.  The sending unit corrected the error and transferred the correct patient of which allowed the nursing assistant to continue with her documentation and care of the patient.  I am proud of this nursing assistant's great catch, in that even though she had begun to chart in the incorrect patient's medical record, she stopped herself, questioned her assessment with another colleague and the error was caught before further issues occurred.  Thank you Ty'Ieshia Fluellen, NA for having the courage to speak up.
