Case Manager with Caring Compassion at its Best!!

We all know some of those special patients. We had one 'very' special patient and family on 7S PCU. Discharge planning was intricate to say the least. Our Case Manager worked so hard to make sure that this patient's discharge to home was as smooth as it could be. Theresa Smith went far above and beyond for this patient & his wife. Working beside her and with this pt for the past 100 days, I have no doubt that Theresa can handle right about anything! I heard somewhere that at the end of every test there is a testimony -so I trust that many a staff on 7SPCU can testify to Theresa's hard work, compassion, advocacy, and professionalism. We all have a renewed level of respect for your tenacity as you have with this patient &family and relentlessly managed to meet his & ‘spouse’s many needs as well as winning her over. We all know her as special but you had a way of communicating with her where you broke down her protective barriers - one layer at a time. Thank you for your long hours of work, the many telephone calls, the meaningful discussions in IDR, the networking and strong communication skills to get all care teams to collaborate in ensuring the safe discharge of this very complex patient.
