Carilion Clinic Advance Care Planning receives City of Roanoke Proclamation in honor of National Healthcare Decisions Day

We are proud to share that the City of Roanoke recently recognized the importance of advance care planning by presenting the Carilion Clinic Advance Care Planning (ACP) Council with a proclamation in honor of National Healthcare Decisions Day.

This proclamation names April 16 as a day to encourage conversations about personal healthcare wishes and the values that guide care at every stage of life. It reflects a shared commitment between the City of Roanoke and Carilion Clinic to support informed and compassionate healthcare decisions for patients, families, and care teams.

The ACP Council sincerely thanks Councilmember Phazhon Nash for presenting the proclamation and recognizing the value of advance care planning in our community. We also appreciate Dr. Suzy Kraemer (Chief Quality Officer for Carilion Clinic), Dr. Jeff Goode (Physician Champion for ACP Council and ED Observation Provider), Ashli Semones (CAPS SR. Director for the PMO and Outcomes Mortality) , and Wendy Davis (Co-Chair ACP Council and CAPS QIF Outcomes Mortality) , who accepted the proclamation on behalf of the ACP Council. Their presence highlights the teamwork and system-wide collaboration behind this important work.

Since launching in 2024, the ACP Council has worked across Carilion to improve education, documentation, and outreach related to advance care planning. This recognition honors that progress and our continued focus on making sure patients’ wishes are known, documented, and respected.

We are grateful to the City of Roanoke for this meaningful recognition and for partnering with Carilion Clinic to promote open dialogue and dignity in healthcare decisions.
