Guidelines for submitting a report of a QA/QI study:    

(*Your submission should be 750 words)    

Please consider and include the following topics in your report:

1. Purpose and Aims

2. Current state of knowledge about project topic by summarizing and synthesizing the available research to provide justification for project.

3. Description of population and benefit

4. IRB determination of not human subjects (use language from IRB letter)

5. Methods and procedures

6. Assessment and outcome measures and data collection

7. Findings

8. How the change improved process, quality or delivery of care at Carilion.

9. Reference List
