Take the Culture of Safety Survey by Sept. 8!
Time is running out! If you haven't taken the Culture of Safety survey yet, you have until Tuesday, Sept. 8 to share your voice.
Safety is foundational to the care we provide and the environment we create for patients, visitors, and employees. Feedback from the Culture of Safety survey helps us understand what's working, identify opportunities for improvement, respond more quickly to concerns, and meet important external requirements tied to patient safety and quality.
The bottom line: Your feedback helps make Carilion safer and more supportive for everyone—and we encourage every employee to participate!
Why Take the Survey?
- Your feedback leads to action. Your input has helped inform system-wide safety and security improvements, including weapons detection, more consistent regional security staffing, and practices that support safer care environments.
- Your voice strengthens patient safety. Feedback and reporting help us recognize risks earlier, prevent harm and share what works across the organization. More than 30% of SafeWatch reports are now classified as near misses or Great Catches, showing our culture of safety in action.
- It is quick and confidential. The Culture of Safety Survey will take roughly 10 minutes to complete and will ask for feedback on topics related to safety and security. Managers will not be able to see your individual responses.
You can access the survey from your personalized email invitation. Please take a few minutes to tell us how we’re doing so we can continue to improve and support you.

Share your feedback in the Culture of Safety survey by Tuesday, Sept. 8.
Your input helps identify opportunities to strengthen system-wide safety, security, and patient care.
There will not be a full engagement survey this year as we prepare to move to more frequent, targeted pulse surveys in FY27.
