Conflict of Interest Questionnaires Coming April 15

As a not-for-profit organization, it’s important that we avoid conflicts of interest. To that end, each year we send a questionnaire to staff who may influence purchasing or operational decisions at Carilion, to identify potential conflicts of interest.

This year’s questionnaires will be sent on Monday, April 15.

Questionnaires will be sent to:

• Carilion-employed physicians and ACPs
• Employees in management roles
• Case managers, social workers and physical, occupational and speech therapists
• Internal Audit employees
• Employees who are considered key personnel for research projects
• Employees who have responsibility for making decisions involving a material purchase of goods or services ($1,000 or more) for Carilion

If you are required to complete the questionnaire, you will also receive an education assignment in Cornerstone on April 15. Here are key things to know:

• The questionnaire will be sent to your Carilion email address from Carilion Clinic Compliance at noreply@coi-smart.com with the subject line "Conflicts of Interest Questionnaire."
• To open the questionnaire, click on the link within the email and log on using your Active Directory username and password.
• Do not click Reply to the message.
• The deadline to complete the questionnaire is Friday, May 31.
