Community Care: A Bridge to Primary Care
For those transitioning their care or searching for a primary care provider, our Community Care practice might be a good fit.
We created this care model for patients who are in need of primary care services, but are unable to get immediate access to a provider. For example, patients who were recently discharged from the hospital and need to schedule a follow-up visit. We may be able to assist patients with finding a Carilion Clinic primary care provider.

Community Care is located at 1229 Third St. in Roanoke and offers same-day and next-day appointment availability for services including:
- Well visits
- Physical exams
- Acute care
- Lab services
- EKG Testing
- Some immunizations
- Chronic condition care
The goal of Community Care is to make sure that everyone is able to receive the care that they need. Please help us spread the word about this option with your friends and the community who might be in need of primary care.
