How to use the following form to send in your pictures:

  • Fill out the information in the form fields.

  • Under Participants, type the full names of everyone in the photograph. 

  • To upload a photo, click Browse, select your file, then click Upload (your photo needs to be less than 2MB). 

  • Read the Social Media and Online Activity policy and the Employee Authorization for Use of Image, Voice or Likeness section of the Employee Guide and click I Agree.

  • Then click Submit and you're done!
