Coverage includes employees, spouse/domestic partner, and dependent children up to age 26.  

Enrollment is guaranteed.  

You can enroll during open enrollment, as a new hire in a full or regular-part time position, or if you experience a qualifying event.

If you no longer work for Carilion, you can continue your coverage by paying premiums directly to LegalEASE. When you separate from Carilion, you will receive a letter giving you guidance on porting the plan and setting up direct pay options.
