As an employee, you can enroll in short- and long-term disability benefits to replace a portion of your earnings if you are not able to work due to an accident, illness or injury. Knowing you will continue to get paid for a period, even if you're not working, can ease your concerns for the future. It's an affordable choice for peace of mind!
Our disability benefit plans are administered by New York Life.
Plan 1- Short-Term Disability
- Short-term disability replaces 60 percent of your eligible earnings, paid to you weekly after you have been out of work for at least 14 days.

Plan 2- Short-Term Disability Buy-Up
Short-term disability replaces 60 percent of your eligible earnings, paid to you weekly after you have been out of work for at least 7 days.
- This means you use less PTO before your disability benefit payments begin.
- The premiums for the buy-up plan are higher than the premiums for the base (14 day) plan. Click on the Contribution Summary to compare premiums between the two plans.

Short Term Disability and Maternity Benefits
If you are on leave due to childbirth and have enrolled in Short Term Disability benefits, you are eligible to receive STD benefits for the 12 weeks directly post-birth. Should you choose to return to work early prior to the 12 weeks, the STD benefit payable would be offset by other income benefits received.
- If the employee’s disability begins at delivery, there is no elimination period and benefits start as of date of delivery.
- If the employee is already disabled on date of delivery and has not yet met the STD elimination period, the STD elimination period ends at the delivery date. *Late enrollment penalty may apply for disability waiting period prior to delivery.

If you do not enroll when you first become eligible to enroll, you may have to provide evidence of good health to be covered and/or wait longer for your disability benefit payments to start.
- Long-term disability replaces 60 percent of your eligible earnings, paid to you monthly after you have been out of work for at least 150 days.
- Payments may continue until you are no longer considered disabled, possibly until Social Security normal retirement age or later depending on your age on the date of disability.
- If your long-term disability insurance ends because you leave Carilion or change to part-time or flex status, you may be eligible to continue your coverage by converting it to an individual policy. Some limits may apply to the amount you can continue through conversion. Contact New York Life at 800-441-1832 for more information about converting your long-term disability coverage.

- Contact your manager if you need to take a leave of absence.
- Contact New York Life to begin the claim process. You may contact New York Life by phone at 888-842-4462 or go to www.mynylgbs.com to file a claim online.
- You will be asked to provide information such as your name and Social Security number, the last day you were at work, information about your condition/reason for disability and the contact information for your physician.
- New York Life will make up to three attempts to contact your physician to obtain the information necessary to process your claim. We encourage you to contact your doctor and request their assistance in responding to the request on your behalf.
- Once New York Life has all of the necessary information to process your claim, it takes about a week to process the information and make a claim decision. You will be notified of the decision or if additional details are needed.
