Rich Text
You may continue your coverage if it is lost because of a reduction in your hours of employment or the termination of your employment for reasons other than gross misconduct.
- Once your separation has been processed, your COBRA information packet will be sent to you automatically from PayFlex/Inspira Financial. You have 60 days from the date your coverage ends or from the date the notice of your election rights is sent to you, whichever is later, to choose whether to continue your coverage.
- Contact Human Resources when you are dropping coverage for a dependent due to a change in their dependent eligibility. The dependent may be eligible for continued coverage.
- Changes in your coverage due to dropping a dependent may be made through Workday.
- COBRA premiums are paid monthly.
- If you drop a dependent during Open Enrollment, they will not be eligible for COBRA enrollment.
The length of time COBRA coverage may continue depends on the reason you or your dependent’s coverage ended.
You and/or your dependents may continue coverage for 18 months if you:
- Change from full time to part-time employment and are working less than 48 hours per pay period.
- Are laid-off or discharged.
- Leave work voluntarily for reason other than disability.
You and/or your dependents may continue coverage for 29 months if:
- You leave work because you are totally disabled and eligible to receive Social Security benefits.
- You or a covered family member becomes disabled during the first 60 days of COBRA coverage.
Your dependents may continue coverage for 36 months if:
- You die while you are employed by Carilion.
- Your children reach the maximum age limit for coverage under the plan.
- Your spouse or children are no longer eligible for coverage because of separation or divorce.
- You choose Medicare as your primary carrier.*
